Reporting Forms

The reporting forms (§4b AMG and transitional regulation) can be sent either as a printed copy by post or as a pdf file by email.

To document the reported case, the pdf data can be saved on the reporting party's server. In the case of a follow-up notification, the PEI number assigned in the initial notification should be entered in order to enable rapid allocation.
